All specimens must be collected, labeled, processed and transported according to the procedures of Unilab and the manufacturers of the supplies used for collection. If there is an issue, a Unilab representative will contact you to try to get it resolved as quickly and easily as possible. The following are a list of criteria that may cause a specimen to be rejected or a test cancelled:
* Incorrect specimen type for the test(s) ordered
* Friday shipments without the boxes “Saturday Delivery” and “No Signature Required” checked resulting in a Monday delivery
* Improperly labeled specimens
* Insufficient volume to conduct the test(s)
* Incorrect specimen container
* Specimen container damaged, or contaminated
* Incomplete or incorrect test requisition
* Specimen arrived in expired container
* Compromised specimen – examples include: hemolysis, lypemic, clotted specimens
* A requisition was not included with the specimen
* A specimen was not included with the requisition
